Need phone line install help

I am having problems with my DSL modem and think it could possibly be the old telephone wire (red and green) and wanted to hook up CAT 5 wire from the NID to the DSL modem. It should be a very simple job. The wire will go into the basement and then to the first floor living room up through holes in the wood floors.
